2.6 Participating in the Conversation
If there is one idea worth carrying forward from everything we've explored so far, it is this:
You are not separate from your biology.
Nor are you simply a passive observer of it. You are one of its participants.
Throughout this installment, we've followed the remarkable journey of a beta cell. We've watched it sense nutrients. It interpret hormones. It responds to neighboring cells. Adapt to stress. Alter its priorities. And, when necessary, reorganize its behavior in the service of survival.
Along the way, another realization quietly emerged. The beta cell was never acting alone.
Every decision it made reflected information arriving from the larger organism.
The liver.
The gut.
The immune system.
The nervous system.
The extracellular matrix.
The bloodstream.
Its neighboring cells.
Each contributed another voice to an ongoing biological conversation. The beta cell simply responded to the world it experienced.
So do we.

The Human Participant
It is tempting to imagine ourselves standing outside this conversation. Our current healthcare system has prioritized a sense of distance between the person and their health experience. As in, your condition is something beyond you and your ability to handle it. Disease is something for the professionals. Specialized degrees and protocols. We are left watching it unfold, trying to manage it from a distance.
In reality, we have been part of it from the very beginning. Every choice we make becomes another piece of information interpreted by living tissues. Food becomes hormones.
Movement becomes mechanical and metabolic signals.
Sleep becomes circadian instruction.
Stress becomes neuroendocrine communication.
Relationships become measurable changes in immune and nervous system activity.
Medications become new biological messages.
Even rest carries information.
Every experience contributes to the informational landscape our cells inhabit. Whether we recognize it or not, we are continually participating in the conversation.
Agency Is Participation
The word agency is often misunderstood. It is sometimes mistaken for complete control. Biology offers no such promise. Living systems are far too complex. No individual controls every variable. No one chooses their genetics.
Their childhood environment.
An unexpected infection.
A stressful season of life.
Or the moment they receive a diagnosis.
Agency begins after those realities exist. Agency means recognizing that although we do not control every voice within the conversation, we remain one of them.
Our participation matters.
Not because it overrides every other influence. But because biology continually integrates information from every participant. One additional voice rarely changes an entire symphony overnight. Yet every instrument contributes to the music.
Reading the Terrain
Throughout this series, we have used the word terrain to describe the biological environment surrounding our cells. Perhaps a more complete definition has gradually emerged.
Terrain is not simply nutrition. Nor toxins. Nor inflammation. Nor the microbiome.
Terrain is the total informational environment experienced by a living system. Everything participates.
Nutrients.
Hormones.
Light.
Sleep.
Movement.
Thoughts.
Microorganisms.
Immune signals.
Mechanical forces.
Medications.
Relationships.
Time.
Every one of these becomes information interpreted by biology.No single factor explains health. No single factor explains disease. Together, however, they create the context from which biological decisions emerge.
From Guessing to Exploration
Living with a chronic illness often feels like guessing.
Trying another diet.
Another medication.
Another supplement.
Another exercise routine.
Another laboratory test.
When I first started doing self-experimentation related to beta cell health, it was overwhelming. All of the possible therapies, supplements, treatments and trends. Each promised a certain outcome but without a cohesive blueprint. Everything felt random. Ultimately in the same direction, but disjointed (and disappointing).
After a year's worth of attempts coinciding with the necessary legwork and research attached to them, a more cohesive picture began to emerge. There was a system at play. And I noticed that the randomness I saw before, when viewed through the framework of systems biology, started to organize itself.
With the understanding of a larger system (with multiple subsystems at play), guessing became exploration.
Not because uncertainty disappears. But because exploration has direction.
Instead of asking:
"Which intervention is the answer?"
We begin asking:
"Which conversation is this trying to influence?"
"Which biological constraint might this relieve?"
"Which transition requirement does this support?"
Each experiment becomes an opportunity to better understand our own terrain. Success teaches us. Lack of success teaches us. Both become information. Curiosity gradually replaces frustration. Exploration replaces guesswork.
Healing as Greater Coherence
Throughout history, healing has often been described as the disappearance of disease. Living systems suggest something richer. Healing may instead represent the gradual emergence of greater biological coherence. When dealing with as many systems as the human body presents, the "gradual" part of "gradual emergence" implies degrees of healing. Coherence is not a black or white state.
Sometimes that coherence includes complete recovery. Sometimes it does not.
A person may still require insulin while experiencing healthier immune regulation.
Improved sleep.
More resilient metabolism.
Greater physiological flexibility.
Reduced inflammation.
Renewed energy.
A stronger sense of well-being.
Healing and cure are not always identical. Both deserve our attention.
The Conversation Continues
Perhaps the most hopeful realization of all is this:
Biology never stops listening.
Every day, your cells continue gathering information.
Every day, they reassess priorities.
Every day, they ask what is most reasonable under the conditions they perceive.
The story is never frozen. It is continually being revised. Some conversations grow quieter. Others become stronger. Entirely new conversations emerge. Life itself is an ongoing process of interpretation and adaptation.
A Different Relationship With Disease
When we first encounter a diagnosis, it is easy to feel as though biology has become something happening to us. A defunct pancreas, an overactive immune system, a virus, a food group... Something outside of me has rebelled at my expense.
The perspective developed throughout this installment suggests another possibility. We remain participants in an extraordinarily complex living system. While we may not determine every outcome or understand every conversation, we are never absent from them.
Every meal.
Every night's sleep.
Every walk.
Every period of recovery.
Every medication.
Every meaningful relationship.
Every moment of stress.
Every act of self-care.
Each becomes another sentence in an ongoing dialogue with the organism we inhabit. This realization does not guarantee a particular destination. It offers something equally valuable.
It restores agency. Not the agency of certainty. The agency of participation.
You have a say in how this turns out.
Current Working Model
One possible interpretation of type 1 diabetes is that it represents an evolving biological state maintained through countless interacting conversations occurring across the organism.
Those conversations are not fixed. They continually adapt to changing information. While no individual can control every aspect of this process, each person remains an active participant within it.
Health therefore becomes more than the pursuit of a single therapy or a single explanation. It becomes the ongoing practice of cultivating conditions that encourage greater biological coherence.
The goal is no longer to search for one perfect intervention. It is to understand the terrain, recognize the conversations shaping it, and participate thoughtfully in the continual process through which living systems change.
Looking Ahead
Throughout this installment, we have explored how biology makes decisions.
One question now remains.
